Fill out the form and fax to 416.512.1077. Be sure to include a valid email address so that we can send you a registration confirmation.
Cancellation, Substitution and Refund Policy: For attendee refunds or downgrades, send written notice to MMPI Canada via fax 416.512.1077 no later than May 25, 2012. For requests received by May 25, 2012, your registration fee (or the difference in fees) will be refunded, less a $75 handling charge; No credits are given for late notices or “no shows”. Registrants may send a substitute attendee up to and including the day of the event. MMPI Canada reserves the right to rescind any registration, and in that event will return 100% of the registration fee.
